Student Forms

Forms can be completed and signed digitally by doing the following:

  1. Open in Adobe Acrobat (not browser)
  2. Save to computer
  3. Complete form
  4. Click on the Signature field
    1. If you have not created a digital signature, follow the instructional prompts to do so.
    2. If you have a digital signature on your computer:
      1. Enter the password that was set previously, then click Sign
      2. Update the saved file
  1. Email the completed form

Academic Variance Form – This form is to be used when requesting substitution of one required class for another, or when requesting that a class meet or partially meet, a General Education area or program elective requirement. This form IS NOT to be used when seeking transfer course pre-approval. For transfer course pre-approval, go to the Transfer Course Information page.

Challenge Course by Certification – This form allows you to request CBU course exemption or credit for agency, entity or professional organization certification received.  Approved forms are processed in the semester requested.  Be sure to pay particular attention to tuition or fees that may be applied based upon your specific request.  See your Student Success Coach for more information.  Your Student Success Coach can be found at www.calbaptist.edu/FMSSC.

Declaration of Major, Minor, Concentration, Catalog Year Form - This form allows you to add, drop or change your academic program.  When completing this form, your advisor can be found at www.calbaptist.edu/FMA.
