For First Time Applicants and Applicants Changing Schools

This form must be completed by both new and re-admit students and is recommended to be completed either upon acceptance or as part of the admission application/re-admit process.    

For First Time Applicants

  1. Complete VA Form 22-1990 Application for Education Benefits (or VA Form 22-5490 Application for Survivors' and Dependents' Educational Assistance for Chapter 35 applicants),
  2. Complete the CBU Request to Begin Veterans Educational Benefits,
  3. Submit a copy of the Certificate of Eligibility or a copy of DD Form 2384 (Notice of Basic Eligibility) if you are applying as a Reservist, and finally
  4. Submit both completed applications along with either the Certificate of Eligibility or the DD Form 2384 to the Veterans Certifying Official in the Office of the University Registrar. The Veterans Certifying Official will then create a file for you and submit your application to the VA.

For Applicants Changing Schools

  1. Complete VA Form 22-1995 Request for Change of Program or Place of Training (or VA Form 22-5495 Request for Change of Program or Place of Training Survivors' and Dependents' Educational Assistance for Chapter 35 applicants),
  2. Complete the CBU Request to Begin Veterans Educational Benefits,
  3. Submit a copy of the Certificate of Eligibility or a copy of DD Form 2384 (Notice of Basic Eligibility) if you are applying as a Reservist, and finally

  4. Submit both completed applications along with either the Certificate of Eligibility or the DD Form 2384 to the Veterans Certifying Official in the Office of the University Registrar. The Veterans Certifying Official will then create a file for you and submit your application to the VA.

For Applicants Participating in the Yellow Ribbon Program

Students wanting to participate in the Yellow Ribbon Program at California Baptist University must complete and submit CBU's Yellow Ribbon Program Application. In order to participate in this program you must meet the eligibility requirements set forth by the Veteran's Administration Office. These requirements may be found online at http://www.benefits.va.gov/gibill/.  The completed application should be submitted to the Office of the University Registrar.
